Before enrolling into Mountain Health Trust, you must qualify for Medicaid Health Insurance. You can find out if you qualify for Medicaid by calling or visiting your county Department of Health and Human Resources Office. The telephone number for this office is in your phone book, or call 1-888-983-2645, or click here.

You may qualify for Medicaid even if you do not receive food stamps or other kinds of assistance. You must speak to a caseworker at the Department of Health and Human Resources Office to find out if you qualify.

When you hear that you have been approved for Medicaid, you will then receive a monthly medical card. If you have other insurance, such as Medicare or veteran's coverage, or someone in the family is covered through their job, please tell the caseworker.

Your caseworker will also explain to you the services that are covered by Medicaid. Please notify your caseworker immediately of any changes in your income or family size.

Once you qualify for Medicaid, you will receive a packet of information asking you to choose an MCO. If you have questions after you have read the packet, please call 1-800-449-8466. As a Medicaid consumer, you have certain rights and responsibilities. They are:

Rights:

You cannot be discriminated against because of sex, race, color, religion, nationality, ancestry, age, political affiliation or disability.

Your information will be held in strictest confidentiality.

You have a right to appeal any decision with which you disagree. To appeal, visit your local Department of Health and Human Resources; or call the Office of Client Services at 1-800-642-8589; or write to the Bureau of Medical Services, Room 251, 350 Capitol Street, Charleston, WV 25301-1509; or phone 394-558-1700.

You have a right to know why your medical bill was not paid. If this happens, you should call your caseworker at the Department of Health and Human Resources or call the Office of Client Services at 1-800-642-8589. You may also call the Consumer Claims and Review Unit at 1-800-633-4340.

Responsibilities:

You must report any changes in income or living arrangements.

You must be honest in your application for Medicaid.

You must keep all appointments with doctors, dentists, clinics or laboratories. If you cannot keep an appointment, please let that person know right away.

You must not pay for medical services that are covered by Medicaid. The doctor, dentist or other service provider will bill Medicaid directly.
