How to Apply:
You will receive a packet of information that is for consumers in your county. If you click on Plan/Provider Wizard, you will
see a map and a list of counties. Click on your county to see the available Managed Care Organizations.
In the enrollment packet you will find:
- Enrollment forms for each eligible family member. The forms are preprinted with the required information. Please
review the information to make sure that it is correct.
- Provider directories for each plan in your county.
- Brochures/fliers explaining the services.
- A postage-paid envelope for you to use when you return the enrollment form(s).
- A letter encouraging you to keep your review date.
Please turn the enrollment form over. You will find a Health Needs
Assessment. Please indicate any special health needs you and your family may have.
After you have marked the MCO you want, place the form in the postage-paid envelope (no stamp is necessary) and mail it to us.
Or you can enroll by phone, 1-800-449-8466. The call is toll free. The Enrollment Specialists are available from 8 a.m. until
8 p.m. every weekday to answer any questions you may have or to take your enrollment. If you call after hours or on weekends,
please leave a message with your phone number and we will return the call the next business day.
Please read the cover letter in your enrollment packet. If you do not make a choice within 30 days, you will be assigned.
This means that we will make the choice for you. The choice will appear on your medical card.
